What I accept referrals for

• Dental implants: single teeth and short-span bridges, with 3D scan-based planning.
• Porcelain veneers: planned with photos and a preview of the proposed shape.
• Composite bonding: chips, wear, small gaps and edge reshaping.
• Smile design: combined whitening, bonding, veneers and gum contouring.

Your patient stays your patient

I carry out only the treatment you refer for. I write to you after the consultation with the agreed plan, and again when treatment is complete. Your patient then returns to you for examinations, hygiene and all routine dentistry.

How it works

1. Referral: complete the short form below. Please discuss the referral with your patient and get their agreement first.
2. Contact: we contact your patient to arrange a consultation.
3. Consultation: examination, photos, scans where needed, and a written plan with all options and costs. A copy of the plan goes to you.
4. Treatment and hand-back: treatment report to you, and your patient returns to your care.

Radiographs and records

Please do not attach radiographs or clinical records to this form. Once we have your referral, we will tell you how to send any images securely.
